A Clinical Study of the Tibial Pilon Fractures



Abstract
The intra-articular fractures of the lower end of the tibia, so called tibial Pilon fractures, are the severe injuries to the ankle joint and it offers difficult challenges managing in because of variable degrees of metaphyseal disruption,
articular
damage and malleolar displacement.
It has been treated in a number of manners, but the best results have been occurred when stable internal fixation is accomplished using plates and screws. Basic steps in reconstruction are restoration of anatomic length of the fibula with plate
fixation, reconstruction of the tibial articular surface, cancellous bone grafting of the metaphyseal defect and early motion of ankle after buttress plating.
The results of treatment of 18 cases were analysed at the Department of Orthopaedic Surgery, college of medicine, Inha University from may 1989 to September 1993.
@ES the results were as follows
@EN 1. The most frequent type of fracture was type II according to Ruedi and Allgower's classification.
2. The operative treatment was performed with open reduction and internal fixation in 17 cases.
3. The overall good and fair results were 83.3%.
4. The extremely important factors in determining the complication and prognosis are degrees of fracture displacement and comminution.
